What is Chronic Care Management?
Chronic Care Management (CCM) is coordinated care outside of regular office visits. A detailed personalized care plan is developed outlining how your care will be coordinated, your health goals, and challenges. We will assist with any barriers to obtaining medications, medical care or basic needs.
Consent: CCM requires informed written consent and cost-sharing. Patients should also understand that only one provider can deliver CCM services so they should only consent to one provider to provide services.
Cost Sharing: Patients with Medicare often have 20% responsibility for charges. That would be the same for this program. Patients with secondary insurance often have no out-of-pocket responsibility.
Is CCM for me?
As we get older our risk for health complications increases. This could be due to genetics or lifestyle. We are here to navigate these conditions with you. Chronic Care Management (CCM) is a voluntary program for Medicare-eligible patients who have two or more chronic conditions. Chronic conditions ear medical conditions both mental and physical that last longer than a year. These conditions require ongoing treatment and affect quality of life.

Chronic Care Management helps ease the burden of managing multiple conditions on your own. Our program care coordinators are trained to help you identify goals that wil help you with your biggest frustrations.
Coordinators are equipped with information to educate you on these conditions you face every day. This information wil provide tools necessary ot help you understand your conditions and how best to manage them safely.
Community Health Workers
You will be contacted by KU Community Health Workers who will be there for you. They can help with the following and more:
- Understanding where you are in your health journey
- Improving patient access and understanding of health, and social services
- Enhancing the relationship between community members and KU health providers
- Obtaining medications and accessing patient assistance programs
- Setting and achieving personal goals by developing and maintaining a personalized care plan
- Providing health education to increase preventive services and enhance self-management of health conditions.
How can I get started?
Speak with your provider at your appointment. You will discuss the program and sign a consent form stating you understand what the program offers. You will soon be contacted by your CCM care manager.
