New Patients

If you are a new patient, please check in 20 minutes prior to your scheduled appointment. You will need to complete a variety of forms and present your insurance information. We will be happy to submit a claim to your insurance company on your behalf. Please bring your medication bottles with you to your appointment.

Cancellations and No Shows

If you are unable to keep your appointment, we require 24 hours notice of cancellation. To ensure availability of appointments to our patients, we have adopted a “no-show appointment” policy. If you miss three appointments, your physician may review your records to determine if you’ll continue to be seen at our clinic.

Non-Covered Services

You are responsible for services that are denied by Medicaid or other insurance as non-covered services.

Co-Pay

A copay is a fixed dollar amount that your health insurance requires the patient/guardian to pay upfront for a covered service. Copays are due at the time of service. If you are unable to pay your copay at the time of service, you will still be seen and billed for the copay. We do not turn patients away due to inability to pay at the time of service. For individuals who qualify for the UKSM-W MPA federally funded assistance program, the patient responsible amount may be reduced, including the possibility of a reduced or waived copay. For more information please review our Financial Policy.

Prescription Refills

Call your pharmacy for refills. The pharmacy will fax us the request. Prescription refill authorizations will be processed Monday through Thursday, 8:00 a.m. to 5:00 p.m. ONLY. Allow 48-72 hours turnaround to get prescriptions refilled from your pharmacy.

Hospitals

We utilize area hospitals. Our primary hospitals are Wesley Medical Center and Via Christi-St. Francis, where our hospitalist teams care for our hospitalized patients. Your hospital admission will be determined by your personal preference and/or the requirements of your health insurance company.
